I think maybe there was no device set for combat audio by the game and force switching it to something else, then back to the same device updated it on the game end. Windows > Sound > App Volume and device preferences > Find FFXIV and change the Output (top entry) from whatever it is set to to anything else, then back to what the correct device is.

That's an issue that's been happening since 6.0, I've had this happen everal times in either 6.1 or 6.5 for me. Audio crackling was supposed to be fixed by now.
